Details
Please fill out ALL of the following information to RENEW your Tang Soo Do Mi Guk Kwan Association membership.  If you are renewing multiple family members at one time please fill this form out separately for each student.  After the first student is renewed you may select the discounted multifamily member price of $20 for the remaining members.

Unfortunately, you will need to provide your mailing address twice for each student renewed.  The first address request is for the MGK the second request is for the credit card company.  These addresses could differ therefore we need them separately.

All memberships are good for 1 year from expiration date of previous membership.
