University of California, Berkeley, School of Optometry Purchase/refund policy: All cancellations must be made in writing, either by mail, email, or fax. We do not accept phone cancellations. Cancellations made at least 3 weeks before the program will be refunded 100%. Cancellations made less than three weeks before the program will be refunded 80%. Cancellations made within 3 days of the program or "No Show" will not be refunded. If original payment was made by credit card, your cancellations refund will be credited to your credit card account. If payment was made by check, you must submit the following: 1) a copy of the cancelled check or a copy of your bank statement showing the check has cleared 2) your social security number 3) the address where your refund check is to be mailed.
Acteva's Purchase/refund policy: Refunds and cancellations are provided only at the discretion of the organization hosting or sponsoring this event.
