Registration forms without payment will not be processed. You may also register online at www.fadona.org. Cancellations: No refunds will be provided for cancellations made less than 5 business days prior to the course — consideration will be given when a survey occurs. Alternates from the same facility are allowed. A $25 administrative fee will be charged for each cancellation. There is a $25 charge for all returned checks.
