Florida Health Care Social Workers Association Purchase/refund policy: Written notice of registrant's request to cancel must be made in writing and received prior to July 9, 2010 (a $100 administrative fee will be applied). Please fax your requests to (561) 659-1291. After July 9, 2010, no refunds will be made. However, consideration will be given to facilities who have undergone regulatory surveys during the week of the conference. There will be no refunds for membership dues.
